If you would like to become a DIY+ for Autism client, please fill out and submit our secure, HIPAA-compliant intake form online.  After you submit your form, you will be returned to our payment and terms page, with final instructions for our payment process.

 

 

Once you have filled out and submitted the health intake form, you will be redirected to the credit card payment form.  Your card will not be charged until we set your appointment and ship your remedies.
